We are a family-oriented practice that specialises in a variety of conditions and treatments for all ages including: childhood immunisations, all chronic illnesses, minor surgery, youth health, care for the elderly.

How to enrol?
Step 1 : Submit Required Documents
Email the following items to reception@fourkauri.co.nz or bring them in person:

Valid Identification: Passport or birth certificate with a valid photo ID (no other forms of ID accepted)

Community Service Card (if applicable)

Step 2 : Wait for Processing
Once all forms have been received, please allow 10 business days for the transfer of your medical notes from your previous GP. You will be able to make an appointment after this period.

After your first GP consultation, you will receive a portal form to sign, granting you access to our online portal.

What to expect on your first GP consultation?
Get to know you
Your first appointment will be a double session lasting 30 minutes. This allows the GP to get to know you and review your medical history in detail.

Set your goals
We offer free visits with our health coach and health improvement practitioner.
If you're looking to enhance your mental health or need support with behavior change, inform your GP. They can arrange a free consultation, including possible same-day appointments, to discuss your goals.
Set up your online portal
After your initial consultation, you will receive a form to register for our online portal. Once you submit the form, you will receive a link to set a password for your account. Please ensure you set your password within 24 hours.
Through the portal, you can make appointments, request prescriptions, and view lab results.
