What happens next?
Here’s what to expect after you submit your enrolment form.
Provide your medical history details.
Once you complete your enrolment form, you will be directed automatically to a medical history form so we can better understand your health background.
Wait for medical records transfer (up to 10 days).
We will request your records from your previous GP. This process usually takes around 7–10 days, but timing may vary depending on your previous practice.
Book your first appointment.
Once we have your medical records, you can book your first appointment. This must be a 30-minute consultation so your GP can get to know you. Contact reception to make your appointment, or if you need urgent medication.
Register for the patient portal.
After your first visit, see our reception team, who will help you set up access to Health365, our online patient portal.

New patient registrations will reopen on 5 October 2026 for our incoming GP, Dr Maria Valadez, who joins the practice on 12 October 2026.
Please note: appointments with Dr Valadez cannot be booked until her start date of 12 October 2026.
